We are a team that provides a full range of services. Services include but are not limited to:

With these services, the hospice team cares for the patient's physical, mental, social, and spiritual well being.  A plan of care is created with patients and families and is carried out by the hospice team, caregivers, families, friends, and/ or facility staff in a private residences, assisted living communities, or nursing homes to create the ideal environment and conditions for the patient

Nursing Services

Hospice nurses are skilled in assessing and managing a patient’s symptoms, such as pain. They are trained tp provide hands-on patient care. Skilled listeners, hospice nurses also comfort the family while also teaching them how to take the best care of their loved one.

Chaplain Services

Regardless of a person’s beliefs or religious traditions, hospice chaplains are available to address the spiritual issues that often arise as a patient nears death. The hospice chaplain is there for the patient and the family, honoring and supporting the cultural traditions and values they hold dear.

Social Worker Services

Hospice social workers provide emotional and psychosocial support to the patient and family. They coordinate the logistics of the patient’s care by educating patients and family on community resources available. They work with insurance companies and  government programs (i.e. Veterans Administration and in home support services) to provide the care the patient needs. Social workers also help with finances, funeral planning, or other tasks.  

Bereavement Services

The bereavement specialist addresses both anticipatory grief and loss after death. Hospice families receive bereavement support up to 13 months after a death, including consistent contact, support groups, grief education and one-on-one visits. Hospice is always available to those who’ve experienced the death of a loved one.

Volunteer Services

Hospice volunteers are specially trained in hospice and end-of-life issues to provide compassionate companionship for patients and families or to facilitate their care. Their duties can range from visiting patients for companionship to assisting with groceries to  helping in the office.

Physician Services

Every patient in hospice is under the care of a hospice physician who closely monitors the progression of the patient’s illness, prescribes appropriate medications and coordinates care with other members of the team. Hospice physicians invite a patient’s preferred physician to stay as involved as they wish in the care plan.

Hospice Aide Services

Hospice aides are certified nursing assistants who provide personal care to the patient, such as bathing, dressing or mouth care. They are available to ease the burden on family caregivers by participating in activities such as light housekeeping.

Contracted Partners

Physician's Choice Hospice LLC works with various companies to ensure that the patient is receiving appropriate services (i.e. podiatry, respiratory therapy, wound care), medications, equipment, and supplies that would aid in improving the patient's quality of life.

Pharmacy Services

We work with pharmacies, such as Citrus Valley Pharmacy and EZ Care Pharmacy, to ensure that the patient's medications are delivered to the patient's home in a timely and hassle free manner. 

Durable Medical Equipment Services

Because patients in transition from hospital to home should not have to wait, we work with Montgomery DME to provide same day or next day delivery of medical equipment necessary to keep patients comfortable at home.  We have the medical equipment your hospice care plan may call for, which may include but is not limited to: 

  • Oxygen Concentrator with Emergency Tank 

  • Suction Machine

  • Nebulizer

  • Bedside Table

  • Electric Hospital Bed 

  • Commode 

  • Wheelchair

  • Walker 

  • Shower Chair

Medical Supply Services

We partner with prominent companies such as Schraders and Vitality to provide comprehensive medical supplies, including incontinence supplies such as diapers, underpads, and wipes. 

Skilled Nursing Facilities

We are contracted with various skilled nursing facilities to provide care to patients requiring more frequent skilled care or who call SNFs their homes.
