24/7 Telehealth
We provide telehealth services 24/7 to ensure that you have a direct line to medical advice, support, and hands-on care when appropriate, at any time of the day or night. You may contact your hospice team with the number provided during our office hours, which are from Monday to Friday from 8:30am to 5:00pm. After office hours, including weekends and holidays, you may contact our on-call hospice personnel at (626)986-4247 or (626)653-9003. Whether a patient is facing an emergency or a caregiver has questions, we provide immediate support and medical care, avoiding 9-1-1 calls and emergency trips to the hospital.
Hospice Team
Our hospice team is a specially trained interdisciplinary group consisting of but not limited to the physician, nurse, hospice aide, social worker, chaplain, volunteer, and bereavement specialist. We work closely with our patients and their families to ensure that they receive a custom tailored plan of care to meet all their needs. Hospice team members make sure that patients under their care are comfortable, free from pain, and are able to maintain the highest possible quality of life. We visit the patient wherever the patient calls home—a private residence, assisted living community or nursing home.

We collaborate with our contracted pharmacies, medical suppliers, durable medical equipment companies, wound care doctors, podiatrists, nursing facilities, board and care facilities, assisted living facilities to provide our hospice patients with what they need to help maintain comfort and dignity.
